Who is eligible for HeartShare's Day Services?

HeartShare's Day Services are open to all adults with the developmental disabilities, including those with mental retardation, cerebral palsy, epilepsy, autism spectrum disorders, and neurological impairments. We also specialize in day services for adults with multiple handicaps and/or behavioral challenges.

What activities do consumers participate in?

At HeartShare, we stress the importance of being active participants in one's community. Program participants are involved in jobs and activities that are of interest to them, whether it is stocking shelves at a local deli, serving lunch at a senior center, going to a nursery to choose plants for a community garden, or doing maintenance at a local movie theater. These activities help individuals take the next step towards greater independence. Consumers also take part in recreational activities, such as trips to museums, sporting events, the movies, bowling, and a variety of cultural experiences. A variety of center-based activities supplement those activities that take place in the community.

What services does HeartShare offer in its day programs?

At HeartShare, we offer the following services:

  • Skills training and development
  • Promotion of greater independence
  • A variety of interesting and meaningful volunteer and work experiences in the community
  • Diverse recreational and cultural opportunities
  • Socialization with both peers and the broader community
  • Attention to individualized needs
  • Assistance in understanding and making choices
  • A variety of center-based habilitation services
  • Access to clinical supports as needed, including physical, speech and occupational therapy, as well nursing, psychology and social work

Are there any specialized day services for individuals with autism?

In July, 2006, HeartShare opened a small day habilitation program (PACT) in Brooklyn for young adults who had just graduated from public school and were entering the world of adult day services. PACT offers a higher staffing ratio and a service design that specifically addresses the learning, socialization and communication needs of individuals with autism spectrum disorders.

Does each participant have a Services Plan?

Yes, each program participant has a Services Plan reflecting their individual needs, interests and goals. This plan is reviewed and updated semi-annually at a meeting attended by the program participant, his or her family, and all the members of his or her services team.

What is the cost for HeartShare's programs?

HeartShare's services are provided at no direct cost to Medicaid eligible individuals.

What are the program hours?

HeartShare's Day Services generally operate from either 8:00 am to 2:30 pm or 8:30 am to 3:00 pm, Monday through Friday, throughout the year.

Does HeartShare provide transportation?

Transportation is provided unless the individual is able to travel independently or chooses an alternative form of transportation.

Is there a wait list for Day Services?

At times there may be a wait list for our individual Day Services programs. However, this varies by location and may depend upon the specific needs of the individual requiring services.

How are HeartShare's programs funded?

HeartShare's Day Services are funded by the New York State Office of Mental Retardation and Developmental Disabilities with federal Medicaid support. Services are supplemented by the generous support of our donors.
