Our Programs: Kaleidoscope Program

HeartShare is pleased to offer two new programs for adults with developmental disabilities--Day Hab without Walls and Blended Day Services--which complete the spectrum of day services at HeartShare.

Day Hab without Walls differs from traditional group day habilitation programs in that the program participant does not go to a program site each morning and from there to a volunteer or work site. In this program, the staff member meets the participant at his or her home, and together they travel directly to the volunteer or work site. In addition, there is plenty of opportunity for customization of valued activities and for pursuing areas of specific interest, including fitness, art, cultural activities, and socialization in the community, just to name a few. This program is unique to each individual, and a comprehensive services plan is created in which valued outcomes, goals and services are outlined, undertaken and monitored.

The Blended Day Services Program combines traditional group day habilitation, pre-vocational services and supported employment. These services are blended together to create a weekly schedule of activities, training and work that that best meet the needs of the individual program participant. This program also differs from traditional supported employment program in that the job coach remains with the supported worker at his or her job site until the individual ready to graduate to the next level of less supervised supported work.
