What are HeartShare's Residential Services for individuals with HIV/AIDS?
HeartShare's Residential Housing Program helps individuals and their families find and maintain comfortable living arrangements that best meet their needs. HeartShare has a strong reputation for having clean, well maintained, and safe homes. Our staff works hard to locate an available apartment or house in Brooklyn or Queens that best meets the needs of our residents. Once housing issues are resolved, individuals and their families are able to focus on obtaining medical care, finding employment, and improving their family situation. Additionally, to eliminate isolation, we maintain contact with individuals and their families, and arrange group trips and activities for individuals and their families.
What is HeartShare's Community Follow-Up Program for individuals with HIV/AIDS?
HeartShare's supportive and dedicated staff provides individual and family-centered case management services. By increasing access to HIV-related services, we can help to prevent or delay unnecessary crisis in the lives of individuals and families affected by HIV/AIDS. We use a team approach with a specially-trained case management staff to develop a comprehensive plan, advocate for needed services offered by other providers, and obtain consistent and ongoing access to medical care.
Who is eligible for HeartShare's HIV/AIDS Services?
All individuals living in Brooklyn or Queens who are at risk of HIV/AIDS or have been diagnosed with HIV/AIDS are eligible for HeartShare's Community Follow-Up program. This program serves men and women who are Medicaid eligible, and services are extended to dependent children up to age 21.
To be eligible for our Residential Services, individuals and their families must be referred by the HIV/AIDS Services Administration of New York City's Human Resources Administration.

HeartShare's programs are funded in part by the New York State Department of Health AIDS Institute Initiative's Comprehensive Medicaid Case Management Authority, the New York City Human Resources Administration's HIV/AIDS Services Administration, and the generous support of our donors.
