Our Programs: Medicaid Service Coordination

HeartShare's Medicaid Service Coordinators help individuals with developmental disabilities access vital services and plan for their future. Our program is overseen by established and respected professionals who set high standards for integrity and responsiveness to individuals and families. They keep current on available resources that best match the needs and interests of our service recipients.

Service coordinators work in partnership with the individual, his or her family and other service providers to set goals that are meaningful to the individual and enriching to his or her life, in a process that includes writing an Individualized Services Plan. Our service coordinators invest in developing a relationship with each person they work with to be an effective advocate and support him or her in developing his or her strengths, addressing challenges, and working towards his or her goals.
