Who is eligible for HeartShare Wellness' Services?
HeartShare Wellness specializes in serving individuals who have a developmental disability; however, Wellness also accepts other patients who have health insurance or can pay on a fee-for-services basis. Patients are often referred though other patients or their parents, service coordinators, physicians, or other health professionals. However, a referral is not required for services.
What services are available at HeartShare Wellness?
HeartShare Wellness' services currently include:
- Primary medical care
- Internal medicine
- Dermatology
- EKG screening
- Gynecology
- Hearing test and screening
- Laboratory tests and screening
- Neurology
- Nutrition counseling and evaluation
- Optometry and Vision therapy
- Physical and Occupational therapy
- Podiatry
- Psychiatry
- Psychological testing and counseling
- Pulmonary care
- Speech pathology and therapy
- Social work
- Referrals and linkages are available for other needed health services.

Special ambulette service can be provided for those who are eligible.
What are the hours for the clinic?
Monday through Friday
8:30 am to 5:00 pm
Evening hours are also available on some nights. Please contact our office for information on evening services.
While patients are seen primary on an appointment basis, emergency visits can be accommodated during normal business hours.
What payment is accepted for services?
HeartShare Wellness accepts Medicare, Medicaid and private insurance. Please bring all your insurance cards to each visit. Patients can also pay on a fee-for-service basis at the time they are seen.
