Heartshare exists to remove obstacles, build stability, and enable for greater self-belief, self-advocacy, and independence.
We support two specific populations across all of New York City: children and adults with developmental disabilities; and children and families dealing with the trauma associated with family separation. Each population faces its own distinct set of challenges and obstacles — and we are set up accordingly to meet them with understanding, evidence-based solutions, and interconnected programming.
While the actual work of each focus area varies, Heartshare as a whole is driven by a few guiding principles. These ideas form the foundation of all work — from how we judge our success to how we set ambitious plans for the future.

155 Years of Community
Heartshare has grown to become an integral part of the communities that host our programs and services. After over a century of building trust and improving outcomes, we now proudly serve every corner of the city.

Community Programs
Brooklyn-based community programs — operating out of 4 schools and 3 NYCHA Housing locations — provide safety, academics, recreation and arts enrichment for children, youth, and families.
Residential Programs
Located in Brooklyn and Queens, these programs go beyond simple housing — offering educational coaching, employment training, and wellness support — so that youth and adults can build independence and a belief in their futures.
Integrated Health Services
A suite of on- and off-site services across the City providing skills-based therapy and healthcare management to empower individuals to achieve wellness. Programs strengthen family and social relationships, achieving personal goals and contributing more meaningfully to communities.
Preventive Programs
Family service centers reach all corners of NYC to provide free community-based services to help families in crisis and to ensure their children remain safe in their homes. Staff provide guidance to families on where to find essential resources. We place great importance on keeping families whole — the goal is to not reach the level of foster care.
Foster Care
With homes across all five boroughs, we ensure that children experience a seamless transition to a safe, stable and loving home, whether that means their reunification with biological parents or an adoption. We work with foster parents to provide the necessary support system for all youth in care.
Day Programming
14 community-based day programs operate across Queens, Brooklyn, and Staten Island to help adults with developmental disabilities build life-skills, socialize, create art, volunteer, and prepare for employment.
Education
Four pre-schools and one grade-age school, located in Queens and Brooklyn provide children with developmental disabilities with comprehensive and personalized educational support.
Group Homes & Apartments
50 community-based group homes supporting children and adults can be found in Brooklyn, Queens and Staten Island. 50 supported apartments also exist across those three boroughs — for people looking for more independence.
