Self-Direction in the Community for People with Special Needs

HeartShare’s Community Habilitation Program helps people with special needs living at home to set and fulfill self-directed life goals. Those who live at home can work with a Direct Support Professional of their choosing to develop their personal goals. The DSP meets the program participant at their home and together, they utilize the community setting to develop social skills, to engage in travel training, to pursue leisure activities, to attend medical appointments or to learn how to self-advocate. Due to this one-to-one relationship with the DSP, the program participant has a unique experience customized to pursuing goals that meet the individual’s personal needs and wants. Adults with disabilities who reside in a Community Residence or Individualized Residential Alternative (IRA) apartment also may be eligible to participate in this program in lieu of another day or employment activity. Whether living at home or in a group setting, HeartShare offers support to enable people with disabilities to live an even more independent life.
